It was common back then when they would heckle each other that the hitter would insult the pitcher by calling a shot. Ruth most probably did this when he was pointing at the pitcher (which we know happened) and the rest is history. Afterwards, Ruth is seen pushing in the air as is to say "Don't push me, I'm Ruth, I can do anything." So it is MORE than word of mouth.
