Basically, one team clears the puck from one end to the other and the "penalty" is that the faceoff goes back to the defensive zone of the team that iced the puck. Also, the team that ices the puck has to keep the same players on the ice so fatigue can set in.

And it's when you shoot the puck across both the center red line and the opposing teams goal line (red line that the net is on) but if you're on the power play you are allowed to dump the puck.

If you pass or shoot the puck from behind half ice (meaning, you're on your defensive side of the rink), and none of your teammates touch the puck and it goes all the way down the rink and the other team gets it, it's icing. It's not a penalty, but you have to have a faceoff in your own zone.
