It's not just that Trev is not locating. He's also not putting hitters away. Doesn't help when you work yourself back from 3-0 or 3-1 and the guy fouls off 4 straight 2 strike pitches. Adds to the sky high pitch count and leads to walks or them punishing mistakes.
